This section is for you to document past and present professional experiences that have shaped your current practice.

The following list indicates the type of documents that MUST be included in this section. Please note that they are all mandatory items.

Purpose of the section

•           To give an overview of the therapist's professional practice, including his/her current practice, and history/experience that shape his/her current practice. 

            •           To demonstrate compliance with the registration requirements

Required Documents

•           Resume/CV – complete with up to date information

            •           COTNS registration document(s)

            •           Proof of malpractice insurance
