Annual Renewal applications are sent out around the first week of April each year. They must be fully completed and returned to this office along with proof of professional liability insurance (this proof must included the Certificate Number) and a cheque by May 15th. These must be received in this office on or before May 15th of each year or a late fee will apply. Incomplete forms will be returned and the late fee may apply.
We strongly urge occupational therapists to read all documentation that is enclosed with the Renewal application so no errors are incurred.
If you do not plan to re-register, please send us a letter requesting that you wish to surrender your license at this time. We would like for those who wish to surrender to let this office know in writing by May 15h.
All members of the College must:
- Have proof of professional liability insurance in the amount of $5,000,000.00,
- Have a legal expenses endorsement for the purpose of investigation of complaints by a regulatory body.
- Submit proof of their professional liability insurance with their Annual Registration Form by May 15th each year. This may include a copy of your certificate of professional liability insurance from CAOT (certificate number must be on documentation) or written confirmation of your plan through another agency or from your employer.
