Annual Registration

Annual Renewal letters are sent out around the first week of April each year.   If you do not wish to do on-line registration, please print the form below and complete and return to this office by May 16th this year or a late fee will apply.    Incomplete forms will be returned and the late fee may apply.

We strongly urge occupational therapists to read all documentation that is enclosed with the Renewal application so no errors are incurred.   

If you do not plan to re-register, please send us a letter requesting that you wish to surrender your license at this time.  We would appreciate surrender letters to reach this officein writing by May 16th.

Insurance for Professional Liability

All members of the College must:

  • Have proof of professional liability insurance in the amount of $5,000,000.00.
  • Have a legal expenses endorsement for the purpose of investigation of complaints by a regulatory body.
